You do not have to wait until the yearly Open Enrollment to do this. Simply call the toll-free number located on your insurance card and speak to Customer Service. Request to switch medical groups. If you decide to stay with Hill Physicians and would like me to transfer your records to another physician, please print the form located on our site, and mail or fax to our office. Transfers will be free of charge until June 30, 2011, after which time a fee of $30 will apply. |
