Insurance Plans Accepted
HMO Insurance
We accept HMO insurance from Affinity Medical Group.
Please check your insurance card to verify that your HMO plan is
administered by either of these groups.If it is not, you may call your
insurer to request that you get switched into either of these groups.
They will do this for you immediately and without charge.
PPO Insurance
We accept all PPO insurance as an "out-of-network" provider. This
means we will ask you to pay for your care at the time of service. We will
send a claim to your insurer on your behalf, and they will send you
reimbursement based on your out-of-network benefits.
You may wish to contact your insurer to verify your out-of-network

Medicare
We accept Medicare by medical referral. You need to be referred to us
by your primary care doctor or medical specialist for a medical condition
in which you are seeking a medical consultation for treatment or surgery.
Medi-Cal, Tricare
We do not accept.
